29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):     a) Throughout Facility - on or about October 20, 2016, employees are performing servicing and maintenance work on production machinery, to include mechanical power presses, but the employer did not develop machine specific procedures which clearly identified all of the specific steps to be followed to isolate and disable the machinery in order to safely de-energize the equipment and control the hazardous energy.      The Mal-Ber Manufacturing Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1067418, citation number 1, item number 1a, and was affirmed as a final order on April 13, 2015, with respect to a workplace located at 2115 Byberry Road, Huntington Valley, PA.

29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section:    a) Shop - on or about November 3, 2016, employees were operating multiple overhead bridge cranes to include, but not limited to, the Morris-Wheeler 10 Ton without each crane receiving daily, monthly, or annual inspections.      Abatement Note: Inspections shall be completed per the intervals listed in 29 CFR 1910.179(j)(1)(ii) to evaluate each overhead crane for the defects outlined in 29 CFR 1910.179(j)(2)(i)-(iv) and/or 1910.179(j)(3)(i)-(x). 29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment:     a) Production Area - on or about October 26, 2016, employees were operating the Verson 250 and Warco 50 Mechanical Power Presses which had not received periodic and regular inspections to ensure that all of the parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, anti-repeat feature, and single-stroke mechanism, on each of the presses were in a safe operating condition and in proper adjustment. 29 CFR 1910.217(e)(1)(ii): Mechanical power press(es) were not inspected and tested at least weekly to determine the condition of the clutch/brake mechanism, anti-repeat feature, and single-stroke mechanism:    a) Production Area - on or about October 26, 2016, employees were operating the Verson 250 and Warco 50 Mechanical Power Presses which were not inspected and tested at least weekly to determine the condition of the clutch/brake mechanism, anti-repeat feature, and single-stroke mechanism.      Abatement Note:  In addition to inspecting the equipment, the employer must:  a.  Perform and complete necessary maintenance or repair, or both, on the clutch/brake mechanism, anti-repeat feature, and single-stroke mechanism before operating the press; and   b.  Maintain a certification record of each maintenance task performed under the directed component of the inspection program that includes the date of the maintenance task, the signature of the person who performed the maintenance task, and the serial number, or other identifier, of the power press maintained.    Note to paragraph (e)(1)(ii): Inspections of the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism conducted under the directed component of the inspection program are exempt from the requirement to maintain certification records specified by paragraph (e)(1)(i)(C) of this section, but inspections of the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism conducted under the general component of the inspection program are not exempt from this requirement. Repairs must be made as necessary, and a certification document must be created    The Mal-Ber Manufacturing Company, was previously cited for a violation of this occupational safety and health standard 1910.217(e)(1)(ii), which was contained in OSHA inspection number 1067418, citation number 1, item number 7b, and was affirmed as a final order on April 13, 2015, with respect to a workplace located at 2115 Byberry Road, Huntington Valley, PA.
