Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:       a) Select Industries, 420 S. Cascade Street, New Castle, PA.  On or about October 19, 2016, and times prior thereto, the operator protection device (seatbelt) was not used by the operators when operating the Clark forklift.  The operator protection device was intended to reduce the risk of entrapment of the operator in the event of a tip-over.  Employees operated the forklift for activities including, but not limited to, transporting pallets of the lithographed plates.

On or about October 19, 2016, the horizontal band saw was not guarded to protect the operator from being cut by the unused portion of the saw blade.      b) Select Industries, Inc. 420 S. On or about October 19, 2016, the ingoing nip points and rotating parts of the coating line were not guarded to protect employees from being caught by the moving parts.     c) Select Industries, Inc. 420 S. On or about October 19, 2016, the ingoing nip points and rotating parts of the printing line were not guarded to protect employees from being caught by the moving parts.

On or about October 19, 2016, the pulley and belt on the Chaplet Grinder was not guarded to prevent employees from having their hands/ fingers caught in the rotating parts. The pulley and belt were less than 5 feet from the ground.

On or about October 19, 2016, unused circuit breaker openings in the electrical panel were not closed.  Employees access the electrical panel to turn on and off the lights in the production area.
