29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) For sub contractor employees working at 6710 South Florida Avenue in Lakeland as observed on or about October 18, 2016. Sub contractor employees were working on the 8:12 pitch steep roof installing roof sheathing without the use of a personal fall arrest system. Employees were exposed to an approximate 42 foot fall.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    For subcontractor's employees working at 6710 South Florida Avenue in Lakeland as observed on or about 18 October, 2016.       a) Employees were working on the fourth floor roof while using personal fall arrest equipment without an anchor point. The employees wrapped the lifeline around the stub of a framing member for the truss and connected to itself. The employees were exposed to a fall height of approximately 41 feet 6.75 inches.      b) Employees were working on the third floor balcony while using personal fall arrest equipment without an anchor point. The employees wrapped a lanyard around a framing wall stud and connected to itself. The employees were exposed to a fall height of approximately 21 feet 8.5 inches.
