29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) For sub-contracted employees working at the 3rd floor of building 2 in 1040 Chalet Suzanne Road in Lake Wales, as observed on or about 13 February, 2020.Sub-contracted employees were standing on a makeshift 2"x4" lumber scaffolding while doing wood framing on the balcony. Employees were exposed to an approximate fall height of up to 25 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) For Sub-contracted employees working at 1040 Chalet Suzanne Road in Lake Wales, as observed on or about 13 February, 2020. Sub-contracted employees were observed installing roof framing and sheathing on a 4.5:12 pitch roof without the use of a personal fall arrest system, exposing to an approximate fall height of up to 30 feet.

29 CFR 1926.502(b)(2)(i): Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level; screens and mesh, when used, did not extend from the top rail to the walking/working level and along the entire opening between top rail supports; Intermediate members, when used gbetween posts, were more than 19 inches (.5 m) apart; Other structural members were installed such that there are no openings in the guardrail system that are more than 19 inches (48 cm) wide.   a) For Sub-contractor employees working at 1040 Chalet Suzanne Road in Lake Wales, as observed on or about 13 February, 2020. Sub-contractor employees were observed installing wall framing on the third floor balcony. The mid rail at the balcony was disconnected from one side and resting on the floor while sub-contracted employees were framing the wall on the third floor balcony, exposing to an approximate fall height of up to 21 feet

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  a) For sub-contracted employees working at 1040 Chalet Suzanne Road in Lake Wales, as observed on or about 13 February, 2020. Subcontracted employees were observed using a personal fall arrest system in which the lifeline deceleration webbing was previously deployed and refolded using framing nails to hold it together, exposing to fall hazards.
