29 CFR 1910.136(a): The employer did not ensure that each affected employee used protective footwear when working in areas where there was a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet were exposed to electrical hazards. On or about October 20, 2016 and at times prior thereto, employees were working as pallet repairman while wearing basketball shoes. This hazard exposed employees to struck by and puncture hazards to the foot.

29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure. Employees without instruction in the purpose and use of the energy control procedure routinely repaired a chain on the pallet conveyor, exposing themselves to amputation hazards. This condition directly lead to the amputation of an employees finger.
