29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection was not provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations: On October 21, 2016 and at times prior thereto the spoil piles of excavated materials was not placed at least two feet back on both side of the excavation. An employee worked outside of the trench box near the sloped, excavated sides of the excavation. Note) Certification of corrective action is required for this item.
