29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (Construction Reference 1926.103)(a) Jobsite: On or about October 24, 2016, an employee with facial hair was observed wearing a tight-fitting facepiece that came between the sealing surface of the facepiece and the face exposing the employee to an inhalation hazard.

29 CFR 1910.134(k)(1)(v): The employer did not ensure that each user could demonstrate knowledge of the procedures for respirator maintenance and storage: (Construction Reference 1926.103)(a) Jobsite: An employee was observed wearing a elastomeric half-faced respirator that was contaminated and exposing the employee to a inhalation hazard. The training on the care, cleaning or storage of respirators was not effective as evident by the unsanitary condition observed.
