Big Ox Energy - Siouxland, LLC - 1617 D Avenue - Dakota City, Nebraska 68731 - The employer is failing to ensure that employee(s) who voluntarily wore tight-fitting half mask respirators had been provided the medical evaluation (Appendix C) and health care professional review required by the standard. This hazard last occurred between December 5 to December 23, 2016.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Big Ox Energy - Siouxland, LLC - Dakota City, Nebraska 68731 - Employer is failing to ensure that each authorized employee affixes their own lock or tag to an energy isolating device prior to working on facility equipment including, but not limited to the GEM (Gas Energy Mixing) System. Employees are exposed to electrical, flowing wastewater/chemicals, and mechanical hazards while performing maintenance and repair on this equipment. This violation last occurred during this inspection on or about December 5, 2016 through December 23, 2016 during group maintenance on the GEM unit.
