The employer did not conduct periodic inspections of the energy control procedures (lockout/tagout) at least annually to ensure that the procedures and the requirements of the standard were being followed.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. On or about October 24, 2016, at the Ford Chicago Assembly Plant, all temporary and/or part time employees did not affix personal lockout devices (locks) into the group lockout box or comparable mechanism when servicing machines or equipment. Employees were thereby exposed to the hazards associated with the unintentional startup of the machines or equipment.
