On or about October 25, 2016, at the above addressed jobsite, employees conducting roofing operations were exposed to fall hazards of twenty-four (24) and six tenths (6:10) feet, due to the safety monitor having other responsibilities, such as but not limited to, roofing operations.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. On or about October 25, 2016, at the above addressed jobsite, an employee conducting roofing operations, ten (10) and six tenths (6:10) feet above the ground, were not protected from falling. Advanced Roofing Team Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 929188, citation number 1, item number 1 and was affirmed as a final order on September 10, 2013, with respect to a workplace located at 10 Pheasant Row, Lincolnshire, IL.
