29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:          a) On or about September 5, 2019, employees were exposed to the hazard of falling approximately 14 feet while working in a hoist area at the roof edge without the use of fall protection.           In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.502(f)(4):  Mechanical equipment on roofs was used or stored in areas where employees were not protected by a warning line system, guardrail system, or personal fall arrest system:         a) On or about September 5, 2019, employees were exposed to the hazard of falling approximately 14 feet while operating mechanical equipment (Garlock Roof Cutter) at and along the roof edge without the use of fall protection.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:        a) On or about September 5, 2019, employees were exposed to the hazard of falling approximately 14 feet while throwing material from the roof edge without the use of fall protection or safety monitoring.       Advanced Roofing Team Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 1186460, citation 2, item 1, and was affirmed as a final order on December 13, 2016, with respect to a worksite located at 14355 Highland Ave., Orland Park, Illinois.     Advanced Roofing Team Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 1170371, citation 2, item 2, and was affirmed as a final order on December 13, 2016, with respect to a worksite located at 656 Burnham Ave., Calumet City, Illinois.       No abatement certification or documentation required for this violation.
