29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about October 26, 2016, at facility where employees were exposed to caught-in, and struck-by hazards while adjusting belts on a Norton brand OD grinder without locating and operating all energy isolating devices needed to control the energy to the machine before a guard covering the belts was removed.
