29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: A.) GKN Sinter Metals - DuBois; sintering furnace F23; exit chute - On or about 10/19/16, and at times prior thereto, employees were not provided with hand tools when exposed to moving conveyor belts and pinch point hazards while pulling parts off of a conveyor by hand. A pinch point formed between the out feed table and the exit conveyor belt.

GKN Sinter Metals - DuBois - On or about 10/19/26, the employer did not report a finger amputation/avulsion within 24 hours of the accident. The avulsion/amputation occurred on 10/19/16 at 1:00 am. The company did not report the fingertip avulsion/amputation until 10/24/16 at 4:00 pm.
