29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: On or about October 1,2016, the pit/flooring opening located adjacent to the tandem mill in the Finishing Department at ArcelorMittal in Cleveland, OH was not guarded by a floor cover. The uncovered pit/floor opening measured approximately 27 inches in width, 60 inches in length and had a depth of approximately 21.5- 26 (+) feet.

(A-frame) stepladder used by an employee tasked with servicing the 84 inch tandem mill located in the Finishing department at ArcelorMittal in Cleveland, OH was covered with lubing oils/grease, exposing the employee(s) to a slipping/fall hazard.
