29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an energized circuit breaker panel that had its cover removed. The panel was listed and labeled as 480 VAC. Employees were thereby exposed to electrical hazards.  b. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an un-covered raceway containing energized conductors.  The raceway was servicing a 120 VAC outlet, as outlined in item c. Employees were thereby exposed to electrical hazards.  c. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an open junction box containing energized conductors. The junction box was in-line from the panel listed in item a and the outlet mentioned in item b. Employees were thereby exposed to electrical hazards.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.    On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working on a structural level where floor holes were present. Employees were using jack hammers to de-construct brick walls, and moving debris in buckets and wheelbarrows. The employer failed to ensure that each floor opening was securely covered. Employees were thereby exposed to Slip / Trip / Fall hazards.    No abatement is required for this item.

29 CFR 1926.1053(b)(20): When ascending or descending a ladder, the user shall face the ladder:  On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working from ladders with powered hand tools such as jack-hammers. Employees were ascending and descending the ladder to adjust their working height regularly. Employees were facing away from the ladder surface while maneuvering and working. Employees where thereby exposed to falls of approximately 6' feet onto construction debris.  Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1926.1060(a)(1): The employer shall ensure that each employee has been trained by a competent person in the following areas, as applicable:    On or about October 31, 2016 at the above referenced worksite, employees engaged in demolition and / or Gut-ReHab activities were not trained by a competent person to recognize ladder-related fall hazards in the work area. Employees were exposed to fall hazards while using jack hammers to de-construct brick walls from A-Frame ladders, facing away from the ladder and not maintaining three points of contact.    No abatement is required for this item.
