29 CFR 1926.502(d)(6)(iii): Snaphooks shall not be engaged to a Dee-ring to which another snaphook or other connector is attached.    (a) On or about January 27, 2020, a laborer disposing of demolition debris from a third floor wall opening, connected their horizontal life to a double eye wire cable choker anchor. A 3M Protecta retractable lanyard was also connected to the choker anchor.  This exposed the employee to a fall hazard of approximately 20 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a) On or about January 27, 2020, a laborer was disposing of demolition debris from a third floor wall opening without an adequate fall protection system installed. The employer used a fall arrest system composed of a wire cable anchor, retractable lanyard and 50 foot rope life line that would not limit free fall to no more than six (6) feet or contact the lower level. This exposed the employee to a fall hazard of approximately 20 feet.

29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee.    (a) On or about January 27, 2020, the employer did not ensure employee(s) retained adequate knowledge to erect a proper fall arrest system and/or restraint system to protect the employee from falling or to arrest a fall.  The laborer disposing demolition debris from a third floor wall opening erected a fall protection system composed of a wire cable choker anchor point, attached to a retractable lanyard with an attached 50 foot rope life line.  The erection of this fall protection system would not limit free fall to no more than six (6) feet, contact with the lower level, or prevent the employee from falling over the exposed edge. This exposed the employee to a fall hazard of greater than 20 feet.
