29 CFR 1910.1030(d)(4)(ii)(E):  Reusable sharps that were contaminated with blood or other potentially infectious material were stored or processed in a manner that required employees to reach by hand into the containers where these sharps had been placed:    (a)  On or about October 27, 2016, and at times prior, employees in the Decontamination room of the Sterile Processing Department were exposed to contaminated sharps when they reached in baskets with their hand to remove surgical instruments for cleaning and processing.      Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1904.7(a): An injury or illness that did not meet the general recording criteria was recorded:    (a)  On or about October 27, 2016, and at times prior, the employer recorded the following cases on the 2016 Log of Work-Related Injuries and Illnesses, although they did not meet the general recording criteria:    Case 33 - No outcome or treatment occurred that met the general recording criteria.  Only diagnostic testing was preformed. Case 34 - No outcome or treatment occurred that met the general recording criteria. Case 35 - No outcome or treatment occurred that met the general recording criteria.  The employee was monitored only.
