29 CFR 1910.1001(j)(4)(iv): At the entrance to mechanical rooms/areas in which employees reasonably can be expected to enter and which contain asbestos containing material (ACM) and/or presumed asbestos containing material (PACM), the building owner did not post signs which identify the material which is present, its location, and appropriate work practices which, if followed, will ensure that ACM and/or PACM will not be disturbed: (a) HCA-HealthOne LLC DBA Presbyterian/St. Luke's Medical Center at 1719 East 19th Avenue in Denver, CO 80218: On or about May 18, 2022, and at times prior, the employer did not post signs in the mechanical boiler room to indicate that pipe insulation was ACM/PACM and to identify appropriate work practices to ensure that plant operations employees would not disturb the material.

29 CFR 1910.1001(j)(7)(iv): The employer did not provide to all employees who perform housekeeping work in areas where ACM and/or PACM is present, an asbestos awareness training course covering the health effects of asbestos; and/or locations of ACM and/or recognition of ACM and PACM damage and deterioration and/or requirements in this standard related to housekeeping, and/or proper response to fiber release episodes: (a) HCA-HealthOne LLC DBA Presbyterian/St. Luke's Medical Center at 1719 East 19th Avenue in Denver, CO 80218: On or about May 18, 2022, and at times prior, the employer did not provide asbestos training to facilities employees who performed maintenance/housekeeping activities in the mechanical boiler room, where ACM and PACM was present.
