29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood that completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut: (A) At the 750 Fairview Road jobsite, on or about October 28, 2016 and at times prior to, employee used Dewalt table saw with no guard to rip board, exposing the employee to amputation and lacerations hazards.

29 CFR 1904.39(a)(1): The employer did not report the in-patient hospitalization of an employee within twenty-four (24) hours to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that is nearest to the site of the incident. (A) At the 750 Fairview Road jobsite, on or about October 10, 2016 and at times prior to, employee used a Dewalt table saw with no guard to rip board. The employee's left thumb fingertip was amputated and the employee was hospitalized. The employer did not report the amputation/hospitalization within 24 hours as required. The injury was not reported to OSHA until October 13, 2016.
