(A)  At the 750 Fairview Road jobsite, on or about October 10, 2016 and at times prior to, employee used a Dewalt table saw with no guard to rip board.  The employee's left thumb fingertip was amputated and the employee was hospitalized.  The employer did not report the amputation/hospitalization within 24 hours as required.  The injury was not reported to OSHA until October 13, 2016.
