29 CFR 1910.178(m)(5)(ii): A powered industrial truck shall not be left unattended when the operator is 25 ft. or more away from the vehicle which remains in view, or whenever the operator leaves the vehicle and it is not in view. On or about Jan, 16, 2019 at 1385 Access Road, Covington, Georgia, as the controlling employer, while the operator of the all-terrain forklift was left with the forks raised and working with other employees on the roof, the employees working on the lower level were exposed to a struck by hazard when the forks were raised and the forklift left unattended.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about January 16, 2019 at 1385 Access Road, Covington, Georgia, as the controlling employer three (3) employees installing metal decking were exposed to a fall hazard of approximately 15 feet to the lower level when not wearing fall protection.
