OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards:    (a) On or about 10/21/2016 - at Reactor #7 in Production Building #2, an employee was modifying the reactor to install a gasket at the top manway and operating a hand grinder that generates sparks while Aluminum Chlorohydrate (ACH) was being manufactured in a process reaction that generates flammable hydrogen gas.

29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent:    (a) On or about 10/21/2016 - at Reactor #7 in Production Building #2, a tag was not used to identify the potentially hazardous condition of gaseous hydrogen in the reactor during the batch manufacturing process for Aluminum Chlorohydrate (ACH).

29 CFR 1910.1200(f)(7): The employer did not ensure the written materials were readily accessible to the employees in their work area throughout each work shift:    (a) On or about 10/21/2016 - at Reactor #7 in Production Building #2, the employer used Site Procedure DEC-OPS-001, "Aluminum Chlorohydrate (ACH) Manufacturing" dated September 16, 2016 to identify in the General Notes that "Hydrogen gas is a by-product and generated in the reactors during the ACH process."  The maintenance employees who work on the reactor do not access this Operations procedure and the hazard was not identified on the reactor.
