29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Mixing Room, Donaldson Torit Dust Collector.  On or about October 21, 2016, procedures were not utilized for the control of hazardous energy when employees were assigned cleaning tasks near two rotary air lock valves.  Employees were exposed to amputation and laceration hazards from contact with the moving parts of the rotary air lock valves due to unexpected energization or startup.  No alternative method of protection was available, such as machine guarding.      Abatement documentation is required in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to the following:  (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices and other energy control measures:   Mixing Room, Donaldson Torit Dust Collector.  On or about October 21, 2016, employees performed cleaning of the dust collector and were exposed to amputation and laceration hazards from contact with the moving parts of two rotary air locks on the dust collector due to unexpected energization or startup.  Employees were not provided with adequate lockout/tagout procedures that contained a means to enforce compliance and a specific statement of the intended use of the procedure for such cleaning tasks.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:    (A)  Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  (B)  Each affected employee shall be instructed in the purpose and use of the energy control procedure.  (C)  All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.    Mixing Room, Donaldson Torit Dust collector.  On or about October 21, 2016, all employees who performed cleaning of the dust collector had not received training in the function of the energy control program and the safe application, usage and removal of energy controls for hazardous energy sources such as rotary valves.   Employees were not trained as "authorized" employees and were exposed to amputation and laceration hazards.    Work Area Protection Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 1135795, citation number 1-3c and was affirmed as a final order on September 20, 2016 with respect to a workplace located at 2500 Production Drive, St. Charles, IL 60174.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    Mixing Room, Donaldson Torit Dust Collector.  On or about October 21, 2016, three authorized employees performing cleaning tasks on and around the dust collector did not each affix a personal lockout device to a group lockout device before servicing the equipment.  Employees were exposed to amputation and laceration hazards from the unexpected energization of the rotary air locks on the dust collector.  No alternative method of protection was available, such as machine guarding.    Work Area Protection Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(ii)(D), which was contained in OSHA inspection number 1135795, citation number 1-3d and was affirmed as a final order on September 20, 2016 with respect to a workplace located at 2500 Production Drive, St. Charles, IL 60174.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1904.40(a): The employer did not provide an authorized government representative with copies of records kept under part 1904 within four business hours.    On or about October 28, 2016, the employer did not provide copies of requested injury and illness records as described below to the Compliance Safety and Health Officer.  The employer did not respond to a formal written request for the same information dated November 30, 2016 with a requested response date of December 14, 2016.    1.  Copy of OSHA 300 Log for Work Area Protection Corp's St. Charles establishment for calendar year 2016.      2.  First Report of Injury Form (OSHA Form 301 or Equivalent) for a work-related injury incurred by David Gutierrez on October 21, 2016.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
