29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Work site was located at 12 River Road, Chatham, NJ 07928 Employees were exposed to amputation hazards as the Toggle-Aire Press machine did not have Lock Out / Tag Out procedures that were effectively communicated and understood by the employee using the machine resulting in a significant injury. Violation occurred on or about 10/14/2016.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Work site was located at 12 River Road, Chatham, NJ 07928 Employees were exposed to amputation hazards as the employer did not provide adequate training to employees to aide in the recognition of hazards associated with power machines and the potential for stored energy that could be contained by a complete Lock Out / Tag Out program. Violation occurred on or about 10/14/2016.
