No. 10A-1970: Company yard: On or about October 27, 2016, fire extinguishers were missing from their mounts in the following instances, this condition exposed employees to fire hazards. a) Fire extinguisher in the main shop work section was missing. b) Fire Extinguisher in the equipment and vehicle maintenance section was missing.

29 CFR 1926.405(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures: Company yard: On or about October 27, 2016, three outlets were missing weather outlet covers in the following instances, this condition exposed employees to electrical shock hazards. a) The outlet cover to the receptacle in the main shop work section was missing. b) Two receptacles outlets in the equipment vehicle and maintenance section had outlet lovers missing.
