29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and rebuild  activities where they were exposed to electrical hazards, including shock and burns. Employees were exposed to fall hazards from unprotected openings in a floor a wall and falls from ladders. The employer failed to inspect and / or have a competent person recognize any and all defects and associated hazards. Employees were thereby exposed to electrical hazards, falls from heights, and / or slip / trip / falls associated with uncovered / unguarded floor openings.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:  a. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an energized circuit breaker panel that had its cover removed. The panel was listed and labeled as 480 VAC. Employees were thereby exposed to electrical hazards.  b. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an un-covered raceway containing energized conductors.  The raceway was servicing a 120 VAC outlet, as outlined in item c. Employees were thereby exposed to electrical hazards.  c. On or about October 31, 2016 at the above referenced worksite, employees were engaged in demolition and / or Gut-ReHab activities. Employees were working in close proximity to an open junction box containing energized conductors. The junction box was in-line from the panel listed in item a and the outlet mentioned in item b. Employees were thereby exposed to electrical hazards.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  a. On or about October 31, 2016 at the above referenced worksite, employees engaged in demolition and / or Gut-ReHab activities were exposed to slip, trip, and fall hazards on a structural level where floor holes were present. The employer failed to ensure that each floor opening was securely covered before employees were assigned to use jack hammers to demolish brick walls, and move debris in buckets and wheelbarrows.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
