Machine Shop, Chalmers and Kubeck North, 24-34 Elise Street, Westfield, Massachusetts; On or about November 1, 2016 the machine shop was not kept in a clean and orderly condition including, but not limited to, material blocked access to equipment, metal shavings were on the floor and covering the lathes, and empty plastic trays were stored on top of a barrel of lubricant.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Machine shop, 24-34 Elise Street, Westfield, Massachusetts: On or before September 21, 2016 employees were exposed to amputation hazards on metal lathes where the chuck guards had been removed to work on large parts and not replaced when working on smaller parts. Chalmers and Kubeck was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1085033, citation number 1, item number 1 and was affirmed as a final order on October 26, 2015, with respect to a workplace located at 24-34 Elise Street, Westfield, Massachusetts.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: Chalmers and Kubeck North, 24-34 Elise Street, Westfield, Massachusetts: On or about September 21, 2016 an employee suffered an amputation while operating a lathe at the facility. The local OSHA office was notified by a third party on October 31, 2016.
