29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility- 118 Poorman Street, Tyrone, PA: On or about October 21, 2016, the Quick and Easy- Line 3 was not guarded to protect employees from rotating parts and ingoing nip points of the conveyor; an employee sustained an amputation to his left index finger down to the knuckle while operating the equipment. (b) Facility- 118 Poorman Street, Tyrone, PA: On or about October 31, 2016, employees were exposed to rotating parts and ingoing nip points of the "porcupine" rolls to the vac slab slicer.
