29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:  The employer is failing to ensure that employer are trained about a new chemical hazard in the workplace. Hydrochloric acid, which was previously shipped to the company in a different container which employees were familiar with, was shipped in a similar container that hydrogen peroxide is shipped in. An employee which was not informed of the new chemical container color, used the hydrochloric acid with hydrogen peroxide improperly, creating chlorine gas.  Abatement certification and abatement documentation is required for this violation.  The abatement certification sheet is enclosed.
