29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: a. Warehouse: On or about November 2, 2016, employees were exposed to struck by hazards, in that manufacturers approval was not provided to employees while using the Komatsu 25 forklift (SN 589559A) to move individual marble, granite or other rock materials with the rock clamp. b. Warehouse: On or about October 2, 2016, employees were exposed to struck by hazards, in that manufacturers approval was not provided to employees while using the Hyster XL2 forklift that was used with the spreader bar and synthetic slings to move bundles of materials.
