29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Warehouse: On or about July 13, 2017, and at times prior, the employer exposed employees to struck by hazards, in that the manufacturer's approval was not provided for the Aardwolf FB1-2720 rock clamp, while employees use the Komatsu 25 forklift (serial number 589559A). b) Warehouse: On or about July 13, 2017, and at times prior, the employer exposed employees to struck by hazards, in that the manufacturer's approval was not provided for the Totall T5350924S4 attachment, while employees use the Hyster S120XL2S forklift (serial number D004D04601U). c) Warehouse: On or about July 13, 2017, and at times prior, the employer exposed employees to struck by hazards, in that the manufacturer's approval was not provided for the Caldwell 10S-5-24 spreader, while employees use the Hyster S120XL forklift (serial number D004V09074S). SGM, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1188259, citation number 01, item number 01 and was affirmed as a final order on January 10, 2017, with respect to a workplace located at 2575 Cobb International Blvd, Kennesaw, Georgia, 30152.
