(a)  1003 East King Street, Lancaster Pennsylvania - A employee worked on a flat roof  approximately 25 feet above the ground installing a roof.  The employer did not provide a written certification that the employee received fall protection training.  A employee  received severe injuries when they fell 25 feet to the ground, on or about 8/31/2016.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
