29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103) On or about November 1, 2016, at the above addressed worksite, the employer did not develop and implement a written respiratory protection program for employees required to wear N95 Particulate Respirators while performing rebar replacement operations, and who were also required to work with hazardous chemicals, such as, but not limited to, Silica Crystalline Quartz and Portland Cement. The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.134(c)(1) which was contained in OSHA inspection #936605, Citation 1, Item 2, and was affirmed as a final order on March 18, 2014, with respect to a workplace located at 600 NE 36th Street, Miami, FL 33137.
