29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units:     On or about November 1, 2016, at the above referenced job site, employees were exposed to a 20-foot fall hazard while working on the top of a scaffold platform performing rebar removing/replacing restoration operations adjacent to another scaffold without complete planks.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about November 1, 2016, at the above referenced job site, employee was exposed to a 20-foot fall hazard while working on the top of a scaffold platform, tied off to the scaffold with a personal fall arrest system (PFAS).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about November 1, 2016, at the above referenced job site, employees were exposed to a 20-foot fall hazard while exiting the top of a scaffold platform without the use of an appropriate ladder.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:Note:  This requirement does not apply to float (ship) scaffolds where an edging of 3/4 inch x 1 1/2 inch (2 x 4 cm) wood or equivalent was used in lieu of toeboards.    On or about November 1, 2016, at the above referenced job site, employees who were working below/around supported scaffolds and were exposed to head injuries from hand tools, such as but not limited to rebar, which were being used by employees working on the second and third tier of the scaffold, which were not equipped with toe boards.

On or about November 1, 2016, at the above referenced job site, employees were exposed to a 20-foot fall hazard while working on the top of a scaffold platform did not recognize the missing plank as a potential fall hazard.
