OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from ejected metal pieces while attempting to remove a stuck/hung-up forging from a forging die. a) Forging Hammer Number 85: On or about November 3, 2016, forging hammer operators were exposed to struck-by hazards from broken pieces of metal while using shop made steel rings to dislodge stuck forgings. A metal ring inserted in the No. 85 counter blow forging hammer broke when it was struck by the upper forging die, causing a piece of the ring to eject from the hammer and strike a nearby employee.
