29 CFR 1910.178(l)(4)(ii)(E): Refresher training in relevant topics was not provided to the operator when a condition in the workplace changed in a manner that could affect safe operation of the truck: (a) On or about October 29, 2016, in the Weld Booth Area employees operating the Toyota Electric Forklift Truck (Model 8RH 88RU18, Serial Number 8BRU18-36441) were not provided training on techniques to take when forklifts lose traction on the ground.
