29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept clear from work areas, passageways, and stairs, in and around buildings or other structures: (a) MP Roofing, LCC @ 8501 E. Alameda Ave., Denver, CO 80230: On or before October 31, 2016, the exposing employer did not ensure that scrap lumber was kept clear of the areas where employees walked and worked. The employer allowed 2 x 4's with protruding nails to be placed in work areas, passageways, in and around the building, and in the street. This condition exposed employees to puncture hazards.

29 CFR 1926.252(a): Whenever materials were dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used. For the purpose of this paragraph, an enclosed chute is a slide, closed in on all sides, through which material was moved from a high place to a lower one: (a) MP Roofing, LCC @ 8501 E. Alameda Ave., Denver, CO 80230. On or before October 31, 2016, the exposing employer did not ensure that a trash chute was used for the disposal of roof materials and debris that was dropped 30 feet from the roof eave to the trash container on the ground. This condition exposed employees to struck-by hazards from flying roof materials and other debris.
