29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  A.) Shirin K. Mirzayev dba Super-Roofer, 818-824 State Street, Erie, PA  16501, roof of the structure, on or about November 7, 2016 and times prior thereto:  Employees were performing roofing activities including, but not limited to, screwing roofing fasteners down for the roofing operation.  Employees were not utilizing a means or method of fall protection on the east side of the roof as they were exposed to a fall hazard of greater than 25-feet.

Mirzayev dba Super-Roofer, 818-824 State Street, Erie, PA  16501, roof of the structure, on or about November 7, 2016 and times prior thereto:  Employees were conducting roofing work.  Employees were utilizing a warning line system that did not encompass the entire perimeter of the roof.  The warning system was not present on the roof at the northeast corner of the structure where the work was taking place.

Mirzayev dba Super-Roofer, 818-824 State Street, Erie, PA  16501, roof of the structure, on or about November 7, 2016 and times prior thereto:  Employees were conducting roofing work.  The warning line on the north side of the roof was between 49-inches and 63-inches from the roof edge.

Shirin K. Mirzayev dba Super-Roofer, 818-824 State Street, Erie, PA  16501, roof of the structure, on or about November 7, 2016 and times prior thereto:  Employees were walking/working on the roof of the structure within three feet of two skylights.  Employees were not utilizing a means or method of fall protection when walking/working near the skylights, nor were the skylights protected with covers or a guardrail system.  Employees were exposed to a 15-foot fall hazard.

Mirzayev dba Super-Roofer, 818-824 State Street, Erie, PA  16501, roof of the structure, on or about November 7, 2016 and times prior thereto:  Employees were walking across an elevated walkway from a stairwell area to the roof area where roofing activities were being conducted.  Employees were not utilizing a means or method of fall protection including, but not limited to, guardrail systems, safety net systems, or personal fall arrest systems, when walking across the walkway.  Employees were exposed to a 14.2 to 17.1-foot fall hazard to the roofs below.

The break in elevation between the elevated walkway and the roof where the work was being conducted was 3.4-feet.  Employees were climbing between to elevated walkway and roof without the use of a stairway or ladder.
