29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about October 24, 2016 and at times prior, Campbell Roofing, Inc. as the general contractor did not ensure that the employees of Exteriors Rivera LLC were protected with head protection while working below the roof of a two-story residential house. The employees were exposed to overhead hazards from roofing activities above.

The employees were exposed to eye hazards while using pneumatic nail guns.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about October 24, 2016 and at times prior, Campbell Roofing, Inc. as the general contractor did not ensure that the employees of Exteriors Rivera LLC were protected with fall protection while working on the roof of a two-story residential house. The employees were exposed to approximately 10 to 20 feet fall hazards. Campbell Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 947176, citation number 1, item number 1, and was affirmed as a final order on December 23, 2013, with respect to a workplace located at 1562 Lookout Springs Dr., Colorado Springs, CO 80921.
