29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) On or about October 24, 2016 and at times prior, employees were exposed to overhead hazards from roofing activities above. The employees were not protected with head protection while working below the roof of a two-story residential house.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) On or about October 24, 2016 and at times prior, employees were exposed to eye hazards while using pneumatic nail guns. The employees were not protected with eye protection while using pneumatic nail guns on the roof of a two-story residential house.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about October 24, 2016 and at times prior, employees were exposed to approximately 10 to 20 feet fall hazards while working on the roof of a two-story residential house. The employees were not protected with fall protection system.   Exteriors Rivera LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1179810, citation number 2, item number 1, and was affirmed as a final order on November 23, 2016, with respect to a workplace located at 4772 Gami Way, Colorado Springs, CO 80911.  Exteriors Rivera LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1169553, citation number 1, item number 1, and was affirmed as a final order on September 6, 2016, with respect to a workplace located at 2750 Richmond Dr., Colorado Springs, CO 80922.  Exteriors Rivera LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 989693, citation number 1, item number 1, and was affirmed as a final order on August 29, 2014, with respect to a workplace located at 5100 S. Logan St., Colorado Springs, CO 80121.
