29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, a safety net system, or personal fall arrest systems:    a) Exteriors Rivera, LLC at 1708 Eisenhower Drive, Louisville, CO 80027: On or about 7/24/2018, the employer did not ensure that each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems. Three employees were engaged in re-roofing activities without fall protection on a 2-story residential home. This condition exposed employees to fall hazards in excess of 6 ft.    Exteriors Rivera, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which is contained in Inspection # 1189677 Citation 2, Item 1 and was affirmed as a final order on February 15, 2017, with respect to a worksite located at  5220 Alturas Drive, Colorado Springs, CO 80911.     Exteriors Rivera, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which is contained in Inspection # 1179810 Citation 2, Item 1 and was affirmed as a final order on November 23, 2016, with respect to a worksite located at 4772 Gami Way, Colorado Springs, CO 80911.      Exteriors Rivera, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which is contained in Inspection # 1169553 Citation 1, Item 1 and was affirmed as a final order on September 6, 2016, with respect to a worksite located at 2750 Richmond Dr., Colorado Springs, CO 80922.     Exteriors Rivera, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which is contained in Inspection #989693 Citation 1, Item 1 and was affirmed as a final order on August 29, 2014, with respect to a worksite located at 5100 S. Logan St., Colorado Springs, CO 80121.
