Olive St., Denver, CO 80237: On and before November 8, 2016, the exposing employer did not ensure that employees engaged in roofing a two-story single family residence were protected from fall hazards by a guardrail system, safety net system, or personal fall arrest systems. Condition exposed employees to fall hazards of up to approximately 15 feet.
