29 CFR 1910.184(e)(1): Alloy steel chain slings did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:    On or about November 9, 2016, the employer failed to ensure that the chain slings and hooks they are using in their main shop area with their 3 ton Myers Overhead Crane and their 1 ton Abell-Howe Hoist had permanently affixed identification tags on them which contained the required information.

29 CFR 1910.212(a)(3)(i): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    On or about November 9, 2016, the employer failed to ensure that there was effective guarding in place on the Ryobi Circular Chop Saw located in the main shop area.  Employees who operate the saw are exposed to the blade at the point of operation when cutting various widths of compressed/rolled fiberglass sheets which are being hand fed up to the saw blade.     -

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:    On or about November 9, 2016, the employer failed to provide effective protection to employees who were working side by side, divided only by a red, see through welding curtain, where one employee was welding on one side and the other employee doing regular work duties.  The non-welding employee was not protected from arc flash rays via an appropriate weld curtain or by use of Personal Protective Equipment.
