29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted:      a)  Jobsite, 4221 6th Avenue, Altoona, PA:  On or about November 2, 2016,  3 employees were exposed to fall hazards while installlng a metal roof on a near flat roof that was under 50 feet in width and were not protected from falling approximately 20 feet to the ground.  The employees were not utilizing any type of fall protection, nor was a safety monitoring system in place.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 4221 6th Avenue, Altoona, PA:  On or about November 2, 2016, employees were working on a commercial roof without fall protection and the owner told the csho that he had reviewed fall protection with the employees but did not have written documentation of the fall protection training he had provided to them.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Jobsite, 4221 6th Avenue, Altoona, PA:  On or about November 2, 2016, an employee was exposed to a fall hazard while working from a ladder jack scaffold and was not protected from falling.  The fall hazard to the employee was approximately 17 feet to the ground.
