OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to the hazard of crushed-by hazards:  Location:  11515 Highway NN, Louisiana, MO      The employer is failing to protect employees operating powered industrial equipment (forklifts) from crushed-by hazards.  An employee operating a Toyota forklift, m/n 8F6W15, s/n 35446 during production activities was not wearing a seat belt.

29 CFR 1904.7(a): An injury or illness involving one or more days away from work, was not recorded on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column.  If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.  The employer failed to record workplace injuries on the OSHA 300 Log of Work-Related Injuries and Illnesses for calendar year 2016.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  Location:  Building W at 11515 Highway NN, Louisiana, MO    The electric subpanel mounted on the side of the inventory office at building W was blocked by a tying machine.
