29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: On or about November 9, 2016, and at times prior thereto, in the Machine #6 area and other areas in shop extension cords and hoses were found laid on the floor creating tripping hazards to employees.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 9, 20126 and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances: 1) Throughout the shop, conveyor belt having ingoing nip points did not have guard provided. 2) Hot Stamp #8 through #11 areas, belt and pulley being used as conveyor belt having ingoing nip points did not have guard provided.
