29 CFR 1910.23(c)(1): 29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    a) Production area, tool change; On or about June 13, 2017 and times prior: The employer failed to ensure employees conducting tool change set-up were provided with fall protection when they worked from on top of the tooling head changing hoses which exposed them to the hazards associated with falls of 8 feet 1 inches to the concrete floor below, or parts of the machine.    b) Production area, tool change; On or about June 13, 2017 and times prior: The employer failed to ensure employees  conducting work on the hoppers on line #8 and #9 were protected from falls when removing the hopper which exposed them to fall hazards of 12 feet 1 inch to the concrete floor.
