On or about November 7, 2016 one employee was working on a 3:12 pitch, low slope roof installing new roof shingles with a Hitachi Brand, roof nailer, pneumatic nail gun and was not protected against impact injuries to his eyes because he was not wearing approved impact resistant eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on November 7, 2016, when two employees are working on a residential roof at 2023 McHarry Ave, Jacksonville, FL 32217 without fall protection and were exposed to falls of approximately 8 feet to the ground below. Roofmax Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 981866, citation number 1, item number 1 and was affirmed as a final order on February 4, 2015, with respect to a workplace located at 12762 Avalon Court, Jacksonville, FL 32224.
