29 CFR 1910.23(a)(8)(ii): A floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing.  (a)  FCI, Kitchen, on or about November 8, 2016, and at times prior thereto, where a floor drain (approximately 9 inches x 9 inches x 4 inches deep) did not have a cover to prevent employees from a slip, trip, and/or fall hazard.  Note:  Abatement was completed at time of inspection.
