29 CFR 1910.1001(k)(2): All spills and sudden releases of material containing asbestos were not cleaned up as soon as possible: (a) On or about November 9, 2016, at work site at 118-15 115th Avenue, South Ozone Park, NY 11420 Approximately 40 square feet of water damaged, broken, asbestos containing 9" x 9" floor tile in the basement level corridor outside of the Power Room had not been cleaned up or repaired in an area still accessed by employees. ABATEMENT CERTIFICATION REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: (a) On or about November 9, 2016, at work site at 87-28 109th Street, Richmond Hill, NY 11418 The basement-level doorway leading out to the exterior steps which ascend to the parking lot did not have a sign to indicate that it was an exit discharge. (b) On or about November 9, 2016, at work site at 87-28 109th Street, Richmond Hill, NY 11418 Signs were not present in the basement-level "main disconnect switch room" and "battery room" to indicate the direction of travel to the nearest exit. ABATEMENT CERTIFICATION REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1910.37(b)(7): Each exit sign did not have the word "Exit" in plainly eligible letters not less than six inches (15.2 cm) high, with the principal strokes of the letters in the word "Exit" not less than three fourths of an inch (1.9 cm) wide: (a) On or about November 9, 2016, at work site at 87-28 109th Street, Richmond Hill, NY 11418 The exit sign on the double brown door in the basement-level "day tank" room leading towards the emergency exit was too small, with the letter in the word exit not at least six inches high and three-fourth of an inch wide. ABATEMENT CERTIFICATION REQUIRED PURSUANT TO 29 CFR 1903.19.
