29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a. On or about November 14, 2016, the employer did not ensure that employees utilized energy control procedures to control hazardous energy prior to cleaning the Line 4 Potato Hopper. An employee received a partial finger amputation while cleaning the Line 4 Potato Hopper in that the equipment was not properly deenergized.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a. On or about November 14, 2016, the employer did not ensure employees were trained in the purpose and function of the energy control program. Employees were required to clean out the Line #4 Potato Hopper and were not trained in proper lock out locations, resulting in a partial finger amputation from the rotating parts.
